Asthma, Seizures & Allergic Reactions
Action Plans are individualized plans for your student’s health issue (examples: asthma, allergies, seizures and other chronic health issues). The action plans assist the health care staff in providing safe, individualized care to your student during the school day should the need arise.
Please bring the completed forms to the health office in the building your student will be attending on the the first day of school.
